Herkimer, NY — Herkimer County HealthNet is proud to announce it has been awarded a $30,000 grant from UpMobility Foundation in support of the Unplugged & Uplifted: Family Connection Project.

Launching this fall, the project is designed to reduce family stress, strengthen relationships, and connect families to vital health and social resources. Through rotating Wellness Circles held in trusted community spaces, families will benefit from screen-free engagement opportunities, emotional wellness education and caregiver peer support, on-site Medicaid screening and social care navigation to connect families with housing, food, and behavioral health services.

“This grant is a powerful affirmation of our mission to improve the health and wellness of families in Herkimer County,” said Elyse Enea, Executive Director of Herkimer County HealthNet. “By partnering with UpMobility Foundation, we can meet families where they are, reduce barriers to care, and provide the tools and support needed for healthier, stronger futures.”

The program will serve families throughout the county, addressing both medical and social determinants of health in a holistic, community-driven model.
