MVHS Conducting Survey for Little Falls Office

Dear Community Member,

We are partnering with MVHS to undertake a Health Equity Impact Assessment (HEIA) as part of the planned relocation of the Little Falls Medical Group from 500 E Main St. to 175 W Main St. The New York State Department of Health’s HEIA process ensures that community voices guide how services evolve, how access is improved, and how health equity is upheld.

We are requesting your input through the feedback form to understand:

• How the relocation may impact you or your family
• What improvements you would like to see at the new location
• How MVHS can improve the overall quality of healthcare in the community
